The Planning Committee selected the Theme of “Transforming Health Systems to Optimize Individual and Population Health” for the 7^th Annual Conference on the Science of Dissemination and Implementation. Now, more than ever before, the integration of evidence-based practices and service delivery has the capacity to impact healthcare delivery and population health. The Abstract Submission topics and conference structure has been planned to focus on the highest priority science – current and future -- to help realize this goal.
